General Internal Medicine

Primary care for fever, sore throat, cough, influenza/pneumonia, palpitations, shortness of breath, dizziness; plus allergies and lifestyle diseases (diabetes, hypertension, dyslipidemia, hyperuricemia), insomnia, hyperhidrosis, headaches, and weight change
Diabetes and Endocrinology

Sleep Apnea (SAS)

If you have loud snoring, excessive daytime sleepiness, or morning headaches, you may have sleep apnea (SAS). Untreated SAS is linked to high blood pressure, diabetes ([see Diabetes page]), arrhythmias, and stroke, and increases health and accident risk.

What we offer
• Home Sleep Apnea Testing (1–2 nights) and full Polysomnography (PSG) as needed
• Clear explanation of results and tailored treatment plans
• CPAP (standard therapy) and referral for oral appliance therapy (mandibular advancement device) via partner dentists
Care pathway
Consultation → Home sleep test → Results review → PSG if indicated → Treatment (CPAP/oral appliance) → Regular follow-up

Gastroenterology & Proctology

Digestive and colorectal care—stomach pain, heartburn, bloating, nausea/vomiting, constipation/diarrhea, rectal bleeding; GERD, gastritis/ulcers, functional dyspepsia, infectious enteritis, hemorrhoids, anal fissure, colon polyps. Endoscopy via partner facilities.

General Surgery

Same-day treatment for common injuries and minor procedures: bruises, simple fractures, cuts/lacerations (suturing), abrasions, paronychia/ingrown nails, skin abscesses. Breast symptom evaluation with referral as needed.

Welcome to Nishiazabu Clinic. I’m Tomohiro Hayashi, Medical Director. More than 30 years have passed since we opened our doors in Nishiazabu.

After studying psychology in the United States, I was drawn to medicine and became a physician. I first trained and worked as a surgeon, but my desire to be closer to patients in their daily lives led me to establish this clinic in 1992.

A community clinic’s mission is simple and profound: to protect the long-term health of the people who live and work here, and to provide care that lets them feel secure. Over the decades we’ve grown with our neighborhood—seeing infants become students, and former kindergarteners return with children of their own.

Guided by the belief to be “a good doctor rather than a famous one,” we combine steady, compassionate primary care with the latest advances in medicine. As your family doctor in the community, we will continue to stand by you—today and in the years ahead.

From April 2025, I will see patients on Friday mornings and alternate Saturdays. I practice endocrinology/diabetes and broad general medicine at a Tokyo tertiary hospital, including emergency care, and bring that experience to Nishiazabu Clinic—where I mainly cover general internal medicine, endocrinology/diabetes, and private (self-pay) services.

With training as both a physician and a dentist, I take a whole-person view of health. Time spent living and practicing abroad taught me to communicate clearly and adapt care to each person’s background and needs. I follow current evidence and standard-of-care while staying flexible to your goals—whether that’s CGM-guided diabetes management or everyday concerns.
